Incident Summary:
On December 20, 2004, a 24-year-old male career probie firefighter (the victim) died after he became separated from an interior attack team. The fire was a two-alarm, single-story, residential structure fire. The interior attack team, and another crew assigned as a search team entered through the front door. Entry was delayed due to security bars. This delay allowed the fire to further develop. After approximately four minutes, the crews performed an emergency evacuation from the structure due to intensifying, uncontrollable fire conditions. Immediately after the evacuation, a personnel accountability report (PAR) was called, and soon after, the crews realized that the victim was missing.
